This training course, Incident Investigation for Oilfield Employees, explains the important of incident reporting and the role you play in the investigation as an employee. Incident reporting and investigation is important to help build a culture of safety and reach a zero-incident state, the goal is NOT to cast blame or get someone fired.
This course provides an overview of types of incidents, including accidents, near misses and witnessing unsafe behaviors. Your responsibilities during an incident investigation are explained, including expectations on the information you provide. Incident reporting involves looking at the whole system of safety control so even seemingly insignificant details could be the key to improving safety for yourself and fellow workers.
Finally, the course covers regulatory compliance and the different requirements of the many agencies which could be involved in an investigation. Everyone involved in an investigation should understand company policies, as well as those of the appropriate international, port state and local organizations, such as OSHA or the U.S. Coast Guard.
